Description
Track your medical history. Canada Post’s Personal Vault includes handy personal info cards you can use to record medical history events. You can the record medical history for your whole family, access it from anywhere and share it with trusted contacts.

How it works
- From the Vault tab inside epost, select Add Form and then choose Medical History.
- Fill out the template by entering a name for your medical history event. Enter the date the event occurred and any notes you wish to include.
- Select the plus (+) symbol in the bottom left corner to add another medical history form.
- At the top of the form, in the Form Name field, add a descriptive name for your history.
Example: Aaron’s History - Surgeries - In the Event Name field, enter the specific medical event.

*When you select Try it NOW, you'll be prompted to sign up for a free epostTM account. Once you're signed in to epost, select the Vault tab to complete your registration.
